The success of our hospitalist
program hinges on the communication systems between
our hospitalists and outpatient physicians and our
hospitalists and all of the hospital departments caring
for our patients. Continuity of care can only be achieved
through that communication during the patient's stay.
To succeed in providing high quality care as cost-effectively
as possible, our hospitalist program is centered on
communication. From the time a patient is admitted,
our hospitalist is working closely with case management
and social service, while continuing to report back
to the patient's Primary Care Physician. Due to the
hopitalist's all-day presence, he can respond to emergencies
faster, review tests and monitor changes in a patient's
condition in real time. Any significant changes are
communicated back to the PCP via phone or fax. This
availability, coupled with the hospitalist's knowlege
of sub-specialties, surgical consultants, discharge
planners, clery and others crucial to the inner workings
of the hospital, enable the smoothest hospital stay
for patients - one where they are promptly care for
by fully licensed and certified physicians whose sole
job is to insure the best possible inpatient experience. |
